Graduate School Forms

Doctoral Degree Requirements for Graduation

All Graduate School requirements for graduation are listed below. Unless stated otherwise, you should submit all forms to 316 Johnston Hall. Note: all students must submit an Application for Degree to 200 Fraser Hall or 130 Coffey Hall on or before the first working day of the intended month of graduation.

  1. At least two terms before your Preliminary Oral Exam, file a Degree Program form (also see the Program Checklist listed below in the Department Forms section)
  2. After completion of all Preliminary Written Exams and at least one week before the Preliminary Oral Exam, submit the Preliminary Written Examination Report form
  3. Schedule the Preliminary Oral with the Graduate School at least one week in advance of the exam. The Prelim Oral must take place at least one academic term (15 weeks) before the Final Oral Defense
  4. Within one working day of completion of the Prelim Oral exam, submit the signed Preliminary Oral Examination Report form (This is sent to your committee chair by the Graduate School before your exam)
  5. Complete 24 Thesis Credits (POL 8888) as quickly as possible. You may register for up to 14 credits total each term, so take as many thesis credits as you can while you are on the department payroll.
  6. The term after passing the Preliminary Oral Examination, submit the Thesis Proposal form.
  7. Anytime after the Thesis Proposal has been approved by the Graduate School, you can request your graduation packet from 316 Johnston Hall. The Thesis Reviewer's Report form will be issued at that time
  8. Submit an Application for Degree to 200 Fraser or 130 Coffey by the first working day of the intended month of graduation (Included in praduation packet)
  9. Submit the signed Thesis Reviewer's Report form and schedule the Final Oral Defense at least one week prior to the exam
  10. Return the signed Final Oral Examination Report form no later than one working day following completion of the Final Oral Defense (This is sent to your committee chair by the Graduate School before your exam)
  11. Submit a copy of the thesis abstract and a copy of the thesis (all signed by the advisor[s] ) plus the Microfilm Agreement Form and the Survey of Earned Doctorates by the last working day of the intended month of graduation

Master's Degree Requirements for Graduation

For Master's-track students

  1. File a Degree Program Form with the Graduate School at least one semester before your written and oral exams. Write "Master's" in the "Degree Sought" spot, and check "Plan B" for the Master's Degree option.
  2. Once the Degree Program has been approved by the Graduate School, request a Final Examination Report form and graduation packet before your final oral examination. You must have the Final Exam Form before you report for the exam.
  3. Submit an Application for Degree to 200 Fraser or 130 Coffey by the first working day of the intended month of graduation.
  4. Return the Final Examination Report form by the last working day of the intended month of graduation.

For Ph.D.-track students

  1. File a Master's Degree Program Form with the Graduate School at the same time you file your Ph.D. Degree Program Form. Most of the information will be identical, but write "Master's" in the "Degree Sought" spot, and check "Plan B" for the Master's Degree option.
  2. Once the Degree Program has been approved by the Graduate School, request a Final Examination Report form and graduation packet before your oral preliminary examination. (You can do this at the same time you schedule your oral prelim with the graduate school.)
  3. Submit an Application for Degree to 200 Fraser or 130 Coffey by the first working day of the intended month of graduation. (In this case, "graduation" refers to your receipt of the Master's degree, not your graduation from the Ph.D. program.)
  4. Return the Final Examination Report form by the last working day of the intended month of graduation.

(If you've already had your oral exam, you can still file all this paperwork retroactively to get your Master's degree.)

Political Science Department Forms

  • Conference Travel Reimbursement Form
    (Please submit this form AFTER you have completed your travel.)
    The department's current policy is to reimburse graduate students up to $150 in travel expenses for a conference at which you are a presenter. You may only be reimbursed for one converence per fiscal year (July 1st-June 31st). You will also need to fill out the travel reimbursement form that is available outside Judy's office. This form is just for Dan to pre-approve the reimbursement.

    This form is in Word Perfect. Please download it, fill it out, then email it to Dan Kelliher as an attachment. (Concurrent with turning in your Degree Program Form to him for approval.) He will print out this form on letterhead, sign it, and attach it to your DPF.
